بكار گيري روشهاي تشخيص تكميلي در اسب(1)

اگر يافته هاي مشاهده شده در خلال معاينه فيزيكي به تعين موقعيت روند بيماري كمك نمايد در مرحله بعد مي با يست كار با تمركز بر ان روند ادامه بايد مثلا اگر اسبي با تاريخچه سرفه در معاينه با گوشي نيز افزايش تعداد تنفس و خس خس سينه داشته باشد اقدام بعدي در جهت تشخيص تكميلي بايد در بر گيرنده ازمايش سيتولوژيك خلط و كشت ميكروبي ان ، راديوگرافي از سينه ، تابلوي خون و تعين ميزان فيبرينوژن باشد.
نمونه خون بايد براي تعين نمودار خون ، ميزان فيبرينوژن ، مشخصات شيميايي ارسال گردد. اين ازمايشات به تعين وجود حالت التهابي يا عفوني كمك مي كند. در اسبهايي با التهاب حاد ممكن است افزايش لكوسيت ها ، نوتروفيل ها ، منوسيت ها ، افزايش فيبرينوژن خون تا ظاهر گردد. در حالات التهابي مزمن ممكن است شمارش افتراقي گلبولهاي سفيد به وضعيت طبيعي باز گردد ولي اغلب غلظت فيبرينوژن همچنان بالا مي ماند و همچنين امكان بوجود امدن كم خوني ناشي از بيماريهاي مزمن اسبهاي مبتلا به حالات التهابي مزمن وجود دارد.
.در اسبهاي مبتلا به كاهش وزن ارزشمند ترين ازمونهاي بيوشيميايي سرم عبارتند از:
اندازه گيري كراتينين ، ازت اوره خون (B.U.N) ، انزيمهاي كبدي (سوربيتول دهيدروژناز و گاماگلوتاميل ترنسفراز) ، بيلي روبين ، البومين و گلوبولين.
ازمونهاي مقدماتي ازمايشگاهي براي ارزيابي وضعيت اسب مبتلا به عارضه كاهش وزن
.آزمايشهاي ديگرشامل:
ازمايشهاي تجزيه ادرار، الكتروفورز سرم، ازمايش سلولهاي مايع صفاقي ، و ازمايشات عملكرد جذب معدي – روده اي مي گردد.
.نتايج بدست امده از تستهاي ازمايشگاهي اوليه چگونگي به كار گيري روشهاي تشخيصي بعدي را مشخص مي كند مثلا چنانچه حداقل اطلاعات اوليه افزايش مشخص فعاليت انزيمهاي كبدي را اشكار مي كند ارزيابي بعدي ممكن است شامل اولترا سونوگرافي و بيوبسي كبد باشد.
اگر در مورد اسبي مشخص شده كه حالت ازتمي و ايزوستنوريك دارد، در ان صورت بايد ازمايشات در جهت تشخيص تفريقي اين نارسايي مزمن و نارساييهاي كليه انجام پذيرد و اين ازمايشات بايد شامل اولتراسونوگرافي كليه و اندازه گيري الكترو ليتهايي كه به مقادير جزئي در ادرار ترشح مي شود و ميزان بعضي انزيمها در ادرار باشد.
اگر چنانچه شكي نسبت به وجود يك روند التهابي بر اساس افزايش فيبرينوژن يا تعداد گلبولهاس سفيد (wbc) وجود دارد نمونه گيري و بررسي مايع صفاقي و نيز راديوگرافي از سينه الزامي است نمونه گيري از مايع جنب نيز ممكن است به منظور شناسايي كانون التهاب انجام مي شود .
چنانچه در يك يا هر دوي اين حفرات (سينه و شكم) التهاب چركي تشخيص داده مي شود مايعات نمونه برداري شده از انها را بايد جهت كشت ميكروبي و تعين حساسيت انتي بيوتيكي به ازمايشگاه ارسال كرد .
اما در صورتي كه هيچ مساله غير طبيعي در معاينات فيزيكي و يا بررسيهاي ازمايشگاهي مقدماتي يافت نشد و هيچ نكته اي در خصوص نگهداري و مديريت نامناسب اسب در تاريخچه وجود نداشت انگاه ازمايشهاي اخنصاصي جذب معدي- روده اي براي تعين وجود سندرمهاي سوء جذب توصيه مي شود.
اين ازمايشات معمولا يا شامل گلوگز خوراكي ويا ازمايش جذي گزيلوز مي باشد.
