مشتقات هموگلوبین

مواد مختلفی با هموگلوبین ترکیب شده و ترکیبات زیر را ایجاد می نمایند:
1) اُکسی هموگلوبین یاHb-O2: از اضافه شدن اکسیژن به هموگلوبین ایجاد می شود و به رنگ قرمز روشن است. اگر اُکسی هموگلوبین، اکسیژن مورد نیاز خود را از دست بدهد به آن هموگلوبین احیا شده یا دزوکسی هموگلوبین می گویند که به رنگ قرمز تیره است.
2) کربوکسی هموگلوبین یاHb-CO: ترکیب منوکسیدکربن و هموگلوبین را گویند. منوکسیدکربن گازی است بسیار سمی، بیرنگ و بی بو که در اثر احتراق ناقص مواد سوختنی ایجاد می شود. CO می تواند در محل اتصال مولکول O2 با هموگلوبین پیوند برقرار کند و از آنجاییکه میل ترکیبیCO با هموگلوبین، 200 برابر بیش از میل ترکیبی O2 یا CO2 با Hb است، مولکولCO جای O2 را گرفته و با هموگلوبین ترکیب پایدارتری را ایجاد خواهد کرد. کربوکسی هموگلوبین به رنگ قرمز آلبالویی روشن می باشد. علاوه بر سوختن ناقص مواد، مقادیر بسیار کمی منوکسیدکربن به طور طبیعی در بدن تولید می شود. یعنی در هنگام کاتابولیسم مولکول هِم، از هر مولکول آن یک مولکول بیلی روبین و یک مولکول CO تولید می گردد، بنابراین حتی در حالت طبیعی نیز حدود 2% از کل هموگلوبین بصورت Hb-CO می باشد. لذا در کمخونی های همولیتیک، مقدار Hb-CO نیز افزایش می یابد.
3) متهموگلوبین(Met-Hb) یا همی گلوبین: اگرFe²+ موجود در هِم بهFe³+ اکسید شود، ایجاد Met-Hb خواهد کرد که تمایل برای پیوند با اکسیژن نداشته و لذا قادر به ایفا نقش طبیعی هموگلوبین نمی باشد. در افراد طبیعی غلظت Met-Hb بسیار کم بوده و از حدود 5/1-1% تجاوز نمی کند. گلبولهای قرمز افراد طبیعی دارای آنزیمهایی است که اگر بدلایلی Fe²+ به Fe³+ تبدیل شود، مجدداً آنرا به شکلFe²+ برمی گردانند. با اینحال در بعضی بیماریها غلظت متهموگلوبین در خون بالا رفته و متهموگلوبینمی ایجاد می شود. مهمترین علل متهموگلوبینمی عبارتند از:
§ کمبود آنزیمهای احیاء کننده یعنی متهموگلوبین ردوکتاز (آنزیم دیافورز) که از تبدیل هموگلوبین به متهموگلوبین ممانعت کرده و به عبارت دیگر هموگلوبین را حفظ می کند.
§ نوعی هموگلوبین غیرطبیعی بنام Hb-M وجود دارد که نوعی بیماری مادرزادی می باشد و بعلت تغییر ساختمانی گلوبین، آهن آن بصورت اکسید شده پایدار می ماند.
§ انواع اکتسابی متهموگلوبینمی که بر اثر مواد شیمیایی و داروهای اکسید کننده مانند نیتریتها بوجود میآیند.
متهموگلوبینمی خفیف ایجاد سیانوز (کبودی اندامهای انتهایی بدن) می نماید و این علامت هنگامی دیده می شود که مقدار متهموگلوبین از5/1% بیشتر باشد. اگر متهموگلوبینمی شدید باشد (≥35%)، ایجاد سردرد، ضعف و تنگی نفس نموده و در مقادیر بالای80% کشنده است.
4) سولفوهموگلوبین(SHb) : اگر اتم گوگرد (سولفور) بطور کووالانسی با مولکول هموگلوبین ترکیب شود، تولید SHb خواهد کرد. سولفوهموگلوبین یک هموگلوبین طبیعی است و در افراد سیگاری دیده می شود و نباید آنرا با هموگلوبین S(Hb-S) که نوعی هموگلوبین غیر طبیعی است و در کمخونی داسی شکل دیده می شـــود، اشتباه کرد. این هموگلوبین برخلاف سایر هموگلوبینها غیر قابل برگشت بوده و تا پایان طول عمر گلبول قرمز درون آن باقی خواهد ماند. مقدار SHb در افراد طبیعی کمتر از1% است اما در مواردی مانند مصرف سولفونامیدها و تنفس مواد گوگردی مقدار آن زیاد می شود و ایجاد سولفو هموگلوبینمی می کند که ممکن است بصورت نامحلول رسوب کرده و تولید اجسام هاینز کند.
