متد جراحي در Hip dysplasia

Hip dysplasia يك بيماري يا يك عارضه مفصلي بوده كه در مفاصل coxafemoral سگ رخ مي دهد.مفصل hip از نوع مفاصل ball & socket بوده و ديسپلازي باعث تغييراتي در سر استخوان ران (ball) و يا در حفره استابولوم (socket)مي شود.مراحل اوليه اين بيماري با بي ثباتي در حركات مفصل hip همراه بوده و در توله سگ هاي 8-9 ماهه خود را نشان مي دهد.اين بي ثباتي و سستي در اندام خلفي بدن را وادار مي كند تا براي جبران آن بافت استخواني بيشتري ايجاد كند(osteophytes) و در نتيجه كپسول مفصلي و ليگامان هاي اطراف آن ضخيم تر شده غضروف مفصلي به تدريج از بين رفته و مفصل دردناك مي شود
مداخلات جراحي يكي از روشهاي درماني براي اين بيماري است كه 3 روش متداول براي اين مورد عبارتند از:
1-Triple pelvic osteotomy
2-ّFemoral head & neck excision
3-Total hip replacement

THR= موثرترين روش درماني جراحي بوده زيرا كه مشكل آناتوميكي سگ را بطور كامل رفع مي كند اين تكنيك منحصرا در بيمارستان هاي آموزشي دامپزشكي بكار مي رود زيرا به تجهيزات خاص و تيم جراحي كارآزموده نياز دارد تا بهترين نتيجه را بدهد. اين روش بسيار گران بوده و چيزي حدود 1750 $ به ازاي هر مفصل هزينه در بر دارد(البته خبر خوب اين است كه معمولا اين عمل روي يكي از مفاصل hip انجام مي گيرد و خود به خود فشار از روي مفصل دوم برداشته مي شود و اغلب نيازي به جراحي مفصل دوم نيست)

TPO= فقط در مورد سگ هاي جوان صورت مي گيرد كه كمتر از 18 ماه از زندگي آنها مي گذرد . در اين روش pelvis را از 3 محل مي برند و socket را آنقدر مي چرخانند تا اين مفصل گوي و كاسه اي خوب fit شود سپس پوزيشن جديد توسط plateهاي orthopedic و پيچ محكم مي شود. اغلب جراحان اين روش را بيشتر مي پسندند. قيمت اين روش به ازاي هر مفصل hip حدود 800 $ است.

FHO= Femoral head & neck ostectomy اين يك جراحي نجات دهنده به حساب مي آيد .زماني كه اين جراحي روي مفصلي صورت پذيرد متد هاي ديگر قابل انجام نخواهد بود. در اينجا head و neck استخوان ران را بريده و سگ داراي يك مفصل كاذب مي شود يعني ليگامان ها و كپسول مفصلي و عضلات نقش مفصل را بازي مي كنند . بهتر است اين روش در مورد سگ هايي كه دچار دررفتگي مفصل شده اند يا تغييرات شديد مفصلي دارند اعمال شود.
