مروري بر هموگلوبينوري پس از زايش و ارتباط آن با برخي عوامل مستعد كننده

هدف: حضور بيش از حد انتظار بيماري هموگلوبينوري پس از زايش post parturient haemoglubinuria(PPH) در فاصله زماني اندك و نيز حضور شواهدي دال بر همراهي عوامل محيطي و زمينه ساز در منطقه شهر كرد، نگارنده را بر آن داشت تا با نگرشي تحقيقي - تحليلي به موضوع، مورادي را به رشته تحرير در آورد.
طرح: مطالعه ميداني و نمونه برداري ازدامهاي مبتلا.
حيوانات: 15 راس گاو شيري مبتلا به هموگلوبينوري پس از زايش.
روش: در خلال خرداد ماه تا اواسط تيرماه 1379، پانزده راس گاو شيروار با تاريخچه هموگلوبينوري به درمانگاه ارجاع و با توجه به نتايج حاصل از معاينات درمانگاهي، بررسيهاي آزمايشگاهي و پاسخ به درمان اختصاصي، حضور بيش از حد انتظار هموگلوبينوري پس از زايش در منطقه شهر كرد به اثبات رسيد.
گاوان مبتلا به سندرم هموگلوبينوري به دقت مورد معاينه درمانگاهي و آزمايشگاهي قرار گرفتند. ضمن ثبت پارامترهاي حياتي (تعداد نبض، تعداد تنفس و درجه حرارت بدن)، كليه علايم درمانگاهي يادداشت و نمونه هاي خون (محيطي و وداجي) و سرم حيوانات مبتلا جهت بررسي آلودگيهاي انگلي، تعيين ميزان كمخوني و بررسي ميزان كلسيم و فسفر به آزمايشگاه ارسال گرديد.
تجزيه و تحليل آماري: آمار توصيفي.
نتايج: نتايج حاصله حكايت از عدم ابتلاي گاوان ارجاعي به بيماريهاي تك ياخته اي خوني و عفوني داشت ميزان كلسيم و فسفر سرم گاوان مبتلا به ترتيب معادل 0.45 ± 8.2 و 0.31 ± 2.85 ميلي گرم در دسي ليتر گزارش گرديد.
نتيجه گيري: بيماري هموگلوبينوري پس از زايش به عنوان يك بيماري متابوليك در سطح صنعت گاوداري مطرح بوده و بيشتر در گاوان پرتوليد و در خلال 2 الي 4 هفته پس از زايمان گزارش شده است.
اكثر محققين علت بروز بيماري را، منوط به حضور كمبود دراز مدت فسفر در جيره غذايي دانسته و برخي مشاهده ساير عوارض كمبود فسفر را (قبل از بروز هموگلوبينوري) از شرايط مورد نياز براي پيدايش بيماري مي دانند. بر اساس گزارشات موجود علاوه بر كمبود فسفر، عوامل مستعد كننده و متعدد ديگري همچون؛ كمبود مس و سلنيوم، تغذيه با خانواده خاجيان و مواجهه با هواي سرد در پيدايش هموگلوبينوري پس از زايش دخليند. طي تحقيقاتي بر گاوان تازه زاي زمينگير در منطقه شهر كرد، كمبود فسفر به عنوان عاملي موثر در پيدايش تب شير شناخته شد. همچنين حضور كمبود مس و سلنيوم در منطقه، به عنوان عاملي زمينه ساز مطرح گرديد و شايد بتوان كاهش ميزان بارندگي در سال زراعي، گرمي نسبي هوا در مقايسه با سال گذشته و كاهش كيفيت علوفه مصرفي (به دليل خشكسالي) را از ديگر عوامل مستعد كننده دانست. بر اساس اطلاعات كسب شده از مراجع ذيربط ميزان بارندگي نسبت به سال گذشته زراعي دچار 7 درصد كاهش و گرمي هوا در مدت مشابه با سال گذشته دچار 0.5 درجه سانتيگراد افزايش، شده است. لذا چنين مي توان نتيجه گرفت كه كمبود فسفر همراه با ساير مسايل مطروحه منجر به بروز بيش از حد بيماري هموگلبوينوري پس از زايش در منطقه شده است
