زغال چوب فعال activated charcoal

موارد مصرف : زغال فعال براي درمان غيراختصاصي اورژانس در مسموميت ناشي از اكثر داروها و مواد شيميايي مصرف مي شود. اين دارو مي تواند به بسياري از سموم در معده پيوند يافته وجذب آنها را كاهش دهد، هر چند كه درمسموميت با سيانور كاملا بي اثر و درمسموميت با اسيدهاي معدني ، بازهاي سوزاننده ، اتانول ، متانول و املاح آهن ،اسيدبوريك و مواد پاك كننده نسبتŠ بي اثراست . اين دارو براي درمان اسهال ، سوءهاضمه ، هيپركلسترولميا و كمك به جذب گازهاي روده كه موجب نفخ مي شوند نيزمصرف مي شود.



مكانيسم اثر: به عنوان داروي ضداسهال ،زغال فعال شده بسياري از محركهايي راكه موجب اسهال و نارحتي گوارشي مي شوند، جذب سطحي مي كند. به عنوان پادزهر اين دارو مواد سمي خورده شده راجذب سطحي كرده و مانع از جذب آنها درمعده و روده مي شود.



فارماكوكينتيك :
اين دارو از مجراي

گوارش جذب نمي شود و متابوليزه نيزنمي گردد. راه اصلي دفع آن از طريق

مدفوع است . حداكثر اثر درماني چنانچه دارو طي 30 دقيقه پس از بلع ماده سمي تجويز گردد، حداكثر اثر درماني حاصل مي گردد.



هشدارها:
1ـ مصرف دارو ممكن است موجب بروز تهوع و استفراغ گردد

كه ممكن است در موارد بلع مواد

سوزاننده يا هيدروكربن هاي فرارخطرناك باشد.

2 ـ مصرف طولاني اين دارو براي رفع ناراحتي هاي گوارشي كودكان زير 3 سال ،ممكن است سوء تغذيه ايجاد نمايد.

تداخل هاي دارويي : زغال فعال مي تواندداروهاي موجود در لوله گوارش را جذب كند. جذب استيل سيستئين خوراكي وشربت ايپكا را كاهش مي دهد. لذا درمواردي كه مصرف هر دو دارو لازم است توصيه مي شود كه زغال را فقط پس ازاتمام استفراغ تجويز كرد. زغال را نبايد باشير يا شربت مخلوط كرد زيرا ظرفيت جذب با آن كاهش مي يابد.


نكات قابل توصيه :
1 ـ دارو در ظرف دربسته نگهداري شود.

2 ـ اگر قبلا به مسموم شربت ايپكا داده شده است ، بعد از بروز استفراغ از اين دارواستفاده شود.

3 ـ استفاده طولاني مدت اين دارو به عنوان ضد نفخ توصيه نمي گردد.

4 ـ اين دارو رنگ مدفوع را سياه مي كند،اگرچه اين مسأله از نظر درماني اهميت ندارد.

5 ـ از مصرف همزمان با ساير داروهاخودداري شود.

مقدار مصرف :

بزرگسالان : در مسموميت حاد مقدار

100g ـ 30 گرم (g/kg 2 ـ 1) به صورت سوسپانسيون در آب مصرف مي شود. دردرمان اسهال ، مقدار 520 ميلي گرم مصرف مي شود كه برحسب نياز، هر 30دقيقه تا يكساعت تكرار مي شود، حداكثرمقدار مصرف در اين موارد 4g/dayمي باشد. به عنوان ضد نفخ ، مقدار 4 - 1گرم از دارو سه بار در روز بعد از غذامصرف مي شود.
