موارد مصرف*: دوبوتامين* به*صورت*تزريقي* به*عنوان* يك* داروي* اينوتروپيك* درسكته* قلبي*، جراحي* قلب*، آسيبهاي* عضله*قلب*، شوك* سپتيك* و كارديوژنيك* مصرف*مي*شود.

مكانيسم* اثر: دبوتامين* داراي* اثراينوتروپيك* مستقيم* است* و با تحريك*مستقيم* گيرنده*هاي* بتا ـ يك* قلب*، قدرت*انقباضي* عضله* قلب* و حجم* ضربه*اي* آن* راافزايش* مي*دهد و در نتيجه* باعث* افزايش*برون* ده* قلب* مي*شود. دوبوتامين* فشارزياد ناشي* از پرشدن* بطن* را كاهش* مي*دهد(كاهش* پيش*بار) و هدايت* گره* دهليزي*بطني* را تسهيل* مي*نمايد. افزايش* برون* ده*قلب* ناشي* از اين* دارو، بيش* از اثرات*دوپامينرژيك* آن* سبب* بهبود جريان* خون*كليوي* و ميزان* دفع* ادرار مي*شود.

فارماكوكينتيك*: متابوليسم* اين* دارو كبدي*است* و به*تركيبات* غيرفعال* تبديل* مي*شود.نيمه* عمر دارو تقريب 2 دقيقه* است*. اثردارو پس* از 2ـ1 دقيقه* شروع* مي*شود. اگرسرعت* انفوزيون* آهسته* باشد، زمان*شروع* اثرممكن* است* تا 10 دقيقه* افزايش*يابد. طول* اثر دارو چند دقيقه* و دفع* آن*عمدت از طريق* كليه* و به*صورت* متابوليت*است*.

موارد منع* مصرف*: اين* دارو در بيماران*مبتلا به*تنگي* ايديوپاتيك* هيپرتروفيك*زيرآئورتي* نبايد مصرف* شود.

هشدارها:
1 ـ اين* دارو در صورت* وجودكمي* حجم* خون* يا سكته* قلبي* بايد بااحتياطمصرف* شود.
2 ـ سرعت* انفوزيون* و طول* درمان*به*پاسخ* بيمار بستگي* دارد. اين* پاسخ*باسرعت* ضربان* قلب*، وجود فعاليت*نابجاي* قلبي*، فشارخون*، جريان* ادرار و درصورت* امكان*، اندازه*گيري* فشار وريدي*مركزي* يا فشار گوه*اي* و برون* ده* قلب*تعيين* مي*شود.

عوارض* جانبي*: تاكي* كاردي* و افزايش* قابل*ملاحظه* در فشار خون* سيستولي*بامصرف* مقادير بيش* از حد دارو گزارش*شده* است*.

نكات* قابل* توصيه*:
1 ـ انفوزيون* دوبوتامين*همزمان* با هپارين* و هيدروكورتيزون*،سفازولين*، سفالوتين* و پني*سيلين* از يك*لوله* انفوزيون* توصيه* نمي*شود.
2 ـ دوبوتامين* با محلولهاي* قليايي* مانندبي*كربنات* سديم*، ناسازگاري* دارد.
3 ـ تغييررنگ* محلول* به*رنگ* صورتي*به*علت* اكسيداسيون* جزئي* است*. باوجوداين*، اگر دارو در طول* مدت* توصيه* شده*مصرف* شود، از قدرت* آن* كاسته*نمي*شود.

مقدار مصرف*: مقدار mcg/kg/min 15ـ 2/5انفوزيون* وريدي* مي*شود.
