موارد مصرف*: كلادريبين* در درمان* لوسمي*hairy cell بكار مي*رود و در درمان* لوسمي*لنفوسيتيك* مزمن*، لنفوم*هاي* غيرهوچكيني*و ماكروگلوبولينمي* والدنستروم* نيزمصرف* شده* است*.

مكانيسم* اثر: متابوليت*هاي* فسفريله* شده*كلادريبين* در لنفوسيت*ها و منوسيتهاتجمع* يافته* و اين* سلولها را از بين* مي*برند. اثر دارو در چرخه* سلولي* غيراختصاصي* بوده* و به* ميزان* يكساني* درمرحله* تقسيم* و استراحت* لنفوسيتها اثرمي*كند.
فارماكوكينتيك*: كلادريبين* از سد خوني* ـمغزي* عبور مي*كند. متابوليسم* داروسلولي* است*. در انفوزيون* مداوم* وريدي*نيمه* عمر توزيع* دارو حدود 30 دقيقه* ونيمه* عمر نهايي* آن* 7 ساعت* است*.

هشدارها:
1 ـ دارو به* ميزان* قابل* توجهي* براي* سلولهاي* خون* ساز سميت*دارد.
2 ـ در صورت* بروز سميت* عصبي* مصرف*دارو قطع* شود.
3 ـ شروع* دروه*هاي* بعدي* درماني* باكلادريبين* با احتياط فراوان* و در صورتي*است* كه* اثرات* خوني* دوره* قبلي* از بين* رفته*باشد.

عوارض* جانبي*: كم*خوني* شديد، عفونت*،نوتروپني* شديد، بثورات* پوستي*،ترومبوسيتوپني*، سرفه*، تورم*، فلبيت*،كوتاه* شدن* نفس*، درد معده*، تاكيكاردي*،بي*اشتهايي*، سردرد، تهوع* و استفراغ* وخستگي* غيرعادي* از عوارض* جانبي* مهم* ونسبتا شايع* دارو مي*باشند.

نكات* قابل* توصيه*:
1 ـ در دو هفته* اول*درمان* لوسمي* hairy cell، كم* خوني*،نوتروپني*، و ترومبوسيتوپني* بدترمي*شود، سپس* علائم* بهبود مي*يابد.
2 ـ طي* يك* ماه* اول* درمان* بدليل* كم* خوني*ممكن* است* نياز به* انتقال* خون* باشد. زمان*متوسط براي* بازگشت* هموگلوبين* به*ميزان* طبيعي* 8 هفته* است*.
3 ـ تب* بين* روزهاي* پنجم* و هفتم* درمان* ويا كمي* ديرتر بروز مي*كند و معمولا درلوسمي* hairy cell ديده* مي*شود. درصورت* بروز تب* بيمار از نظر عفونت*بررسي* مي*شود. عفونت* بدليل* كاهش*منوسيت*هاست* و 6-12 ماه* بعد تعدادمنوسيت*ها به* ميزان* طبيعي* بر مي*گردد.
4 ـ زمان* متوسط براي* بازگشت* تعدادپلاكت*هاي* كاهش* يافته* به* ميزان* طبيعي*(بدليل* ترومبوسيتوپني* ناشي* از دارو) 12روز است*. در موارد غير از لوسمي* hairycell، اين* زمان* طولاني*تر بوده* و كاهش*مقدار مصرف* و تكرار دوره* درمان* ممكن*است* نياز باشد.

مقدار مصرف*: در درمان* لوسمي* hairy cellبصورت* انفوزيون* وريدي* مداوم*mg/kg/day 0/1 بمدت* 7 روز مصرف*مي*شود. در اين* بيماري* بطور معمول* فقطيك* دوره* درمان* كافي* است*.
