اتوپوزيد etoposide

موارد مصرف*: اتوپوزيد در درمان*كارسينوم* ريه* و بيضه* به*كار مي*رود.همچنين* در درمان* كارسينوم* مثانه*،لنفوم*هاي* هوچكيني* و غيرهوچكيني*،لوسمي* حاد ميلوسيتيك*، ساركوم* اوينگ* وكاپوسي* مصرف* شده*است*.

مكانيسم* اثر: اتوپوزيد مهاركننده* آنزيم*توپوايزومراز II است* و با اثر بر مرحله* قبل*از ميتوز تقسيم* سلولي*، ساخت* DNAرامهار مي*كند.

فارماكوكينتيك*: فراهمي* زيستي* فرم*خوراكي* حدود 50% است*. انتشار آن* درمايع* مغزي* ـ نخاعي* متغير و كم* است*.متابوليسم* دارو كبدي* بوده* نيمه* عمر داروحدود 7 ساعت* مي*باشد. دارو به*طور عمده*از طريق* كليه* دفع* مي*شود.

هشدار: قبل* از هر بار مصرف* دارو، دهان*از نظر وجود زخم* بررسي* شود.

عوارض* جانبي*: كم*خوني*، كاهش* گلبول*هاي*سفيد يا پلاكت*هاي* خون*، آنافيلاكسي*،استئوماتيت*، كاهش* اشتها، تهوع* و استفراغ*از عوارض* جانبي* مهم* و نسبتا شايع* دارومي*باشند.

نكات* قابل* توصيه*: 1 ـ در مورد شكل*تزريقي*، براي* جلوگيري* از افت* فشار خون*به*صورت* انفوزيون* و در مدت* بيش* از30-60 دقيقه* انجام* شود. از راههاي* ديگرتزريق* يا تزريق* سريع* وريدي* خودداري*شود.
2 ـ تعداد گرانولوسيت*ها 7-14 روز پس* ازتجويز و تعداد پلاكتها 9-16 روز پس* ازتجويز به* حداقل* مي*رسد و در روز بيستم*به*ميزان* طبيعي* برمي*گردد.

مقدار مصرف*:
خوراكي*
در درمان* كارسينوم* سلولهاي* كوچك*ريه* mg/m2/day 70 (به*مدت* 4 روز) تاmg/m2/day 100 (به*مدت* 5 روز) تجويزمي*شود كه* هر 3-4 هفته* تكرار مي*شود.
تزريقي*
در كارسينوم* بيضه* 50-100mg/m2/dayبه*صورت* انفوزيون* وريدي* در روزهاي*اول* و پنجم* درمان* تا 100mg/m2/day درروزهاي* اول* و سوم* و پنجم* درمان* تجويزمي*شود كه* هر 3-4 هفته* تكرار مي*شود. دركارسينوم* ريوي* با سلول*هاي* كوچك*mg/m2/day 35 به*مدت* 4 روز تاmg/m2/day 50 به*مدت* 5 روز تجويزمي*شود كه* هر 3-4 هفته* تكرار مي*شود.
