ايداروبيسين* idarubicin

موارد مصرف : ايداروبيسين به همراه سايرداروهاي ضد لوسمي براي درمان AMLدر بزرگسالان مصرف مي شود.

مكانيسم اثر: ايداروبيسين از طريق واكنش با آنزيم توپوايزومراز II مانع ساخت اسيدهاي هسته اي مي شود.

فارماكوكينتيك : نيمه عمر دارو حدود 22ساعت است . دفع دارو عمدتا صفراوي وبه صورت متابوليت است . ايداروبيسين تابيش از صد برابر غلظت پلاسمايي درسلول هاي هسته دار خون و مغز استخوان تجمع مي يابد.

هشدارها: 1 ـ اغلب بيماراني كه ايداروبيسين را با مقادير درماني مصرف مي كنند به تضعيف شديد مغز استخوان مبتلا مي شوند. اين عارضه ممكن است منجر به مرگ بيمار ناشي از عفونت و ياخونريزي شديد شود.
2 ـ ايداروبيسين ممكن است باعث ايجادعوارض سمي قلبي شود.

عوارض جانبي : عفونت ، تهوع و استفراغ ،ريزش مو، اسهال و كرامپ هاي شكمي ،خونريزي و موكوزيت از عوارض شايع ايداروبيسين هستند.

نكات قابل توصيه :
1 ـ ايداروبيسين بايدآهسته و طي 10-15 دقيقه تزريق شود.
2 ـ ايداروبيسين هنگام تزريق نبايد با سايرداروها مخلوط شود.

مقدار مصرف : به منظور درمان AMLمقدارmg/m2/day 12 به مدت 3 روز همراه باسيتارابين تزريق آهسته وريدي مي شود.ممكن است مقدار 100 mg/m2/day به مدت 7 روز به صورت انفوزيون مداوم و ياmg/m2 25 به صورت تزريق واحد همراه باmg/m2/day 200 به مدت 5 روز به صورت انفوزيون مداوم نيز مصرف شود.
