MELPHALAN

موارد مصرف*: ملفالان* در درمان*كارسينوماي* تخمدان*، پستان*، بيضه* وميولم* مولتيپل* مصرف* مي*شود.

مكانيسم* اثر: ملفالان* داروي* گروه*آلكيله*كننده* است* و در چرخه* سلولي*به*طور غيراختصاصي* اثر مي*كند. اثر اين*دارو عمدتا مربوط به* ايجاد اتصال* بين* دو رشته* مولكولي* DNA و RNA و مهارساخت* پروتئين* است*. همچنين* تا حدي*داراي* فعاليت* كاهنده* ايمني* نيز مي*باشد.

فارماكوكينتيك*: جذب* خوراكي* آن* ناقص* ومتغيير است*. در مايعات* و بافتهاي* بدن*غيرفعال* شده* ولي* در خون* به* مدت* شش*ساعت* فعال* باقي* مي*ماند. نيمه* عمر ان*تقريب 90 دقيقه* است*. دفع* آن* از طريق* كليه*و مدفوع* مي*باشد.

هشدارها: در صورت* وجود انفيلتراسيون*سلولي*، تومور مغز استخوان* و سابقه*درمان* با داروهاي* سمي* براي* سلول* ياپرتو درماني* طي* 3-4 هفته* قبل* از مصرف*ملفالان*، دارو با احتياط فراوان* مصرف*شود.

عوارض* جانبي*: نوتروپني* يا عفونت* وكاهش* پلاكت*هاي* خون* از عوارض* شايع*اين* دارو است*.

نكات* قابل* توصيه*:
1 ـ مقدار مصرف* اين*دارو بايد براساس* نيازهاي* فردي* هر بيمارو مبتني* بر پاسخ*هاي* باليني* و ميزان* كاهش*فعاليت* مغز استخوان*، به*خصوص* به*علت*جذب* غيرقابل* اطمينان* ملفالان*، تنظيم* شود.
2 ـ آشاميدن* آب* كافي* و گاهي* تجويزآلوپورينول* به*منظور جلوگيري* ازنفروپاتي* در بيماران* مبتلا به* لوسمي* ولنفوم* توصيه* مي*شود. همچنين* ممكن* است*قليايي* كردن* ادرار نيز لازم* شود.
3 ـ توصيه* مي*شود كه* در صورت* بروزلكوپني* كاهش* گلبول*هاي* سفيد يا پلاكت*خون* درمان* با ملفالان* قطع* شود. هنگامي*كه* نتايج* معاينات* باليني* و آزمايشگاهي*رضايتبخش* گرديد مي*توان* درمان* را ادامه*داد.

مقدار مصرف*: در ميولم* مولتيپل* مقدار0/15mg/kg/day به* مدت* هفت* روز مصرف*مي*شود كه* يك* دوره* وقفه* تجويز حداقل*سه* هفته*اي* به*دنبال* دارد. مقدار مصرف*نگهدارنده* mg/kg/day 0/05 خواهد بود. درروش* ديگر مقدار mg/kg/day 0/25 براي*چهار روز مصرف* مي*شود كه* يك* دوره*استراحت* 2-4 هفته*اي* به*دنبال* دارد. مقدارمصرف* نگهدارنده* mg/day 2-4 است*. درروش* سوم* مصرف* مقدار mg/m2/day 7 ياmg/kg/day 0/25 به* مدت* 5 روز مي*باشدكه* هر 5-6 هفته* يكبار تكرار مي*شود. تنظيم*مقدار مصرف* بايد طوري* باشد كه* حداقل*اثر كاهنده* گويچه*هاي* سفيد و پلاكت*هابروز نمايد. در درمان* كارسينوم* تخمدان*0/2mg/kg/day به* مدت* 5 روز مصرف*مي*شود كه* هر 4-6 هفته* در صورت*بازگشت* تعداد سلولهاي* خوني* به*مقدارطبيعي* تكرار مي*گردد.
