داروهاي ضدالتهاب غيراستروئيدي

داروهاي ضدالتهاب غيراستروئيدي مانند آسپرين، پيروكسيكام، ايندومتاسين، ايبوپروفن، ديكلوفناك، مفناميك اسيد و ناپروكسن، داروهايي هستند كه براي درمان آرتريت (التهاب مفاصل) و دردهاي عضلاني و نيز ساير دردها مصرف ميشوند. اين داروها در صورتي كه به طور مداوم در طولاني مدت مصرف شوند ممكن است با تداخل در سيستم دفاعي معده و اثني عشر، ايجاد زخم نمايند. مصرف گاهگاهي اين داروها به ندرت باعث چنين مشكلاتي ميشود اما اگر شما قبلاً دچار زخم بوده ايد ممكن است وضعيت شما را بدتر نمايند. اين داروها اثر ضدالتهابي خود در بدن را با كاهش توليد موادي به نام پروستاگلاندين ها ايجاد مي كنند. اگر شما دچار صدمه اي شويد و يا اينكه قسمتي از پوستتان دچار بريدگي شوند، اين منطقه قرمز، گرم و دردناك ميشود. اين وضعيت به خاطر پروستاگلاندين ها ايجاد ميشود كه از سلولهاي آسيب ديده در منطقه اطراف بريدگي ترشح مي گردد. در اكثر قسمتهاي بدن كه پروستاگلاندين ها التهاب را بدتر مي كنند (مانند داخل مفاصل افراد دچار آرتريت) استفاده از داروهاي ضدالتهاب غيراستروئيدي، بسيار موثر مي باشد. با اين حال در معده، پروستاگلاندين ها داراي يك نقش خيلي مهم حفاظتي در لايه مخاطي هستند و مصرف داروهاي ضدالتهاب غيراستروئيدي كه باعث كاهش پروستاگلاندين ه ادر اين منطقه مي شود باعث ضعف اين سيستم دفاعي شده و باعث مي گردد كه در بعضي از افراد، زخم معده يا اثني عشر ايجاد شود.
داروهاي جديدتر ضدالتهاب غيراستروئيدي كه اصطلاحاً به آنها مهاركنندگان سيكلواكسيژناز 2 گفته ميشود اخيراً ساخته شده اند (مثل داروي ملوكسيكام Meloxicam). اين داروها توليد پروستاگلاندين ها را در تمام قسمتهاي بدن به غير از معده كاهش مي دهد و در نتيجه مشكلي در معده يا اثني عشر ايجاد نمي كنند. در آينده، اين داروها ممكن است به كاهش تعداد افراد مبتلا به زخم كمك نمايد. در اينجا لازم است گفته شود كه گرچه داروهاي ضدالتهاب غيراسترئيدي در اكثر افراد باعث بروز زخم نمي شوند، اما اگر شما در هنگام مصرف اين داروها دچار سوءهاضمه ميشويد، حتماً بايد با پزشك خود در اين مورد مشورت نماييد.


علل كمتر شايع بروز زخم
زخم هاي معده و اثني عشر گاهي اوقات در افرادي كه نه آلوده به عفونت هليكوباكترپيلوري هستند و نه از داروهاي ضدالتهاب غيراستروئيدي استفاده مي كنند ديده ميشود. اين زخمها، گرچه معمولاً به درمان دارويي به خوبي پاسخ مي گويند، اما اغلب علت ايجاد آنها مشخص نمي گردد. خيلي به ندرت زخمهاي معده يا اثني عشر ممكن است بر اثر وجود ساير بيماريها ايجاد شود. مثلاً در بيماري كرون (يك نوع بيماري كه ممكن است هر قسمتي از روده را گرفتار نمايد)، سندروم زولينجراليسون (نوعي بيماري كه به علت اختلال هورموني، معده مقدار زيادي اسيد ترشح مي كند) و لنفوم (سرطان سلولهاي خون در ديواره معده) ممكن است زخم ايجاد شود. گرچه امروزه مشخص شده است كه استعمال سيگار به تنهايي نمي تواند باعث بروز زخم شود، اما معلوم شده كه سيگار كشيدن مي تواند باعث بروز زخم شود، اما معلوم شده كه سيگار كشيدن مي تواند باعث اختلال در اليتام زخمها شود. بنابراين به افرادي كه دچار زخم هستند و سيگاري مي باشند، توصيه مي گردد كه دست از سيگار كشيدن بردارند.
